Survey Policy

Please complete the Survey Request form for all SUFU survey requests. If your survey is approved by the committee, you will be notified, and your survey will be sent out according to the SUFU Survey policy.

All survey requests must be made via this survey form or they will not be considered. Surveys not completed with all required information will not be considered.

We cannot guarantee participation in your survey.

  • All requests for membership surveys or postings will be reviewed and approved  by  the SUFU Education Committee
  • There is a limit of 1 email survey per quarter. Other survey requests may be approved for inclusion in the SUFU Newsletter which is emailed to members quarterly.

Membership Survey or Postings Guidelines

  • Membership survey requests or postings that include an SUFU member as the principal investigator or an integral part of the investigative team will be charged be free of charge
  • Membership survey or posting requests that do not include an SUFU member as the principal investigator or an integral part of the investigative team will be charged a $250.00 fee.
  • Survey requests submitted should include:
    • An introductory statement explaining the purpose of the survey and how the results will be used.
    • Estimated time to complete the survey. Must take less than 10 minutes to complete.
    • A link to the survey in the mechanism it will be sent.
  • If questions are ambiguous, the SUFU EC may edit/suggest different wording.

Membership Survey Results

  • The plan for what will be done with the results of the survey needs to be clearly outlined in the proposal request.
