The following events are included in your registration fee. Please mark the boxes below to indicate which events you will be attending.
I will be attending the Welcome Reception on Wednesday, February 29, 2012. I will be attending the Awards Reception on Friday, March 2, 2012.

Payment: SUFU requires payment for registration fees by check, Visa, MasterCard or American Express. Registrations over the phone will not be accepted.

Registration Cancellation & Refund Policy Registration refund requests must be submitted in writing to the SUFU Executive Office by February 10, 2012. All refund requests will be subject to a $100 processing fee. No refunds will be made after February 10, 2012.
